Our students' safety is our top priority. Continue reading below for our physical and concussion policies.
Prior to students being able to practice or compete for an interscholastic athletic team, the following three items must be completed and on file:
1)Annual Physical Exam
- All players are required to have updated physical indicating they are cleared to participate in sports without restrictions. The exam must have been completed within the past 13 months and include a current immunization record.
2)Pre-Participation Head Injury/Concussion Reporting Form [download]
- All players must have this form completed by a parent/guardian prior to the start of each athletic season. It is then reviewed by the school nurse and kept on file. Please note that any head injury a player suffers during the season outside of school must immediately be reported to the team coach, athletic director, and school nurse.
3)Parent/Athlete Concussion Information Sheet [download]
- All players and a parent/guardian are required to complete mandatory concussion training once per academic year. Increased awareness of the signs and symptoms of a concussion can help to protect players ensuring they receive the proper treatment and necessary rest before resuming participation. Please carefully read the forms, sign and date them in the designated areas, then return them to the school nurse to be kept on file.
